We have a downloadable release form which outlines our policies, and also provides you with a means of instructing us as to your wishes in case of emergency. If we have to take your pet to a veterinarian, we will need to know what you want us to do if we are unable to reach you in a timely manner. We always try to contact you or your agent at the numbers you give us, but if a situation is time-critical we may have to make decisions if the contact persons are not available.
